Vicharchika is a type of Kshudra Kushta described in Ayurvedic classics, characterized by symptoms such as Kandu (itching), Srava (discharge), Pidaka (vesicles), and Vaivarnyata (discoloration). It is considered a chronic disease with recurrent exacerbations and remissions. Clinically, Vicharchika can be correlated with eczema or adult-onset atopic dermatitis described in modern dermatology. Contemporary medical management mainly focuses on symptomatic relief using antihistamines and corticosteroids, which may lead to adverse effects on long-term use. Ayurveda emphasizes treatment at the root level by eliminating vitiated Doshas through Shodhana and restoring tissue balance with Shamana therapy. The present case study documents the successful Ayurvedic management of a 57-year-old female patient presenting with chronic recurrent eczematous lesions over both lower limbs. A combination of Shamana and Shodhana therapies along with local treatments resulted in significant clinical improvement. Observations and outcomes were assessed using subjective and objective parameters.
