A Review of Therapeutic Potential of Sthanya Sodhana Mahakashaya

The quality of breast milk (Sthanya) is a pivotal determinant of neonatal health, with Ayurvedic classics emphasizing that vitiated milk leads to various pediatric disorders. This study provides a critical analysis of the Sthanya Shodhana Mahakashaya (breast milk–purifying decoction group), a group of ten medicinal herbs formulated by Acharya Charaka for the purification of breast milk. The analysis focuses on the pharmacodynamic synergy of the constituent drugs-Pata, Mahoushada, Suradaru, Musta, Murva, Guduchi, Vatsakaphala, Kirathathikthaka, Katurohini, and Sariba. The group exhibits a predominant profile of Tiktha–Katu Rasa (bitter–pungent taste), Laghu–Rooksha Guna (light and dry qualities), and Ushna Virya (hot potency). This specific configuration enables a multidimensional mode of action: first, by performing Ama Pachana (metabolic correction and digestion of toxic metabolites) at the level of the mother’s Rasa Dhatu (primary nutritive fluid tissue); second, by providing Srotoshodhana (channel purification) in the Stanyavaha Srotas (lactiferous channels) to remove blockages; and third, by neutralizing the specific Dosha vitiations-Vata, Pitta, and Kapha (functional bio-regulatory principles). Critically, the inclusion of Sariba and Musta provides a necessary cooling counterbalance to the heating herbs, ensuring that the formulation remains safe for the infant’s delicate constitution. The study concludes that the Mahakashaya (major herbal group) acts as a metabolic regulator that optimizes the mother’s lactation physiology. By refining the biochemical and physical properties of breast milk, it serves as both a therapeutic intervention for the mother and a preventive measure against gastrointestinal and dermatological ailments in the nursing infant.
